IT PAYS TO ENROLL IN THE HDHP

By participating in the High Deductible Health Plan (HDHP), you receive tax-free money from the county. Receive $600 for employee-only coverage and $1,200 for employee plus 1 or more coverage (amount is prorated based on the number of pay periods remaining in the benefit plan year). For Open Enrollment, you receive the county contribution with the January 7, 2018 paycheck and it will be deposited in your Optum HSA by January 14, 2018. Contributions will be deposited approximately 2-3 pay periods after you enroll.

GOOD HEALTH PAYS

You can also earn up to an additional $275 for employee-only coverage or up to $550 for spouse/adult designee coverage annually for your HSA by completing the following activities:

  • Annual Preventive Exam $150
  • Personal Health Assessment (PHA) administered through Healthy Lifestyles – $100
  • Annual Flu Vaccination – $25

The wellness incentive is deposited into your HSA quarterly (January, April, July, and October) following the completion of the activity. For example, if you complete your annual preventive exam on April 15, Salt Lake County will deposit $150 into your HSA with your July 22 paycheck.

No documentation is needed to receive the incentive money, but you must participate in Health Lifestyles and consent to share your PHI.
